Doctor - Diabetes Educator (DE)

• Interact with patients following the `Patient Interaction Script`.

• Take verbal consent of the patient before enrolling the patient and initiate the support program with them and document consent.

• Brief patients about the services they will receive under the patient support program Record keeping and maintenance of the Doctor Consent Forms

• Record keeping and maintenance of the Patient Consent Forms (Verbal Consent)

• Educate the patients about diabetic issues and consult

• Record keeping, and maintenance of relevant Files as a part of Good Documentation Practice Ensure completeness and accuracy of data in the Database

• should have consult with the patients regarding diabetic issues, lifestyle, diet chart

• At least 2 years

• Bachelor of Medicine and Bachelor of Surgery(MBBS)

• Age 23 to 30 years
• Only Female

Should have done course of CCD.

Should have experience handling patients with diabetic.
